NDIS Service Agreement

Welcome, I’m really glad you’re here. This agreement sets out the counselling supports Lumi Counselling provides under the NDIS, how payments will be managed, and what we can both expect as we work together.

It’s written with self-managed participants in mind, but it also works if your plan is plan-managed or NDIA-managed. However your plan is set up, the aim is the same: to keep things simple, clear, and supportive.

Take a few minutes to read through the details below. At the end, you’ll be asked to confirm that you’ve read and understood the agreement.

We’ll also go through a signed copy together in your first session, so you’ll have plenty of time to ask questions and feel comfortable with everything before we begin.
