Starting counselling is a big and brave step, and I am so glad you are here! Thank you for trusting Lumi Counselling to walk alongside you. This form is simply here to make your first session smoother by giving me a bit of background about you (or your child/teen).
WHY I ASK THESE QUESTIONS
The questions in this form help me understand your story, your background, and your goals. They also allow me to shape your sessions around your needs and offer the right kind of support from the very beginning.
The form usually takes about 5 to 10 minutes to complete. Everything you share is private and protected under the Australian Privacy Principles. Information will only be shared if required by law or if there is a serious risk of harm to you or others. If you are a parent or guardian completing this form for a child or teen, simply answer what you can on their behalf.
A FEW NOTES BEFORE YOU BEGIN
Once you have finished, I will review your answers and will reach out to you before our first session. There is no pressure to have everything figured out. This is simply a starting point to help me get to know you.
